Change in Revaluation flow

We need to merge two page / forms.

Presently we are showing photo copy download in http://ils.wordpoets.com/student-dashboard/request-photocopy-pdf-list page and to apply for revaluation student has to fill another form https://ilslaw.edu/photocopy-scancopy/revaluation-form

We need to merge this form, just give an button the download photo copy page “Apply for revaluation ” when user click take them to https://ilslaw.edu/photocopy-scancopy/revaluation-form 

I think that is the simple way to implement.
