Exam Fee Form with SBI Online Payment

Need an Exam fee submission form with the following fields 
 
PRN Number 
ILS Unique ID 
Exam Form No 
Full Name 
Class  :  Drop Down : – I BA LLB, II BA LLB , III BA LLB , IV BSL LLB and V BSL LLB , I LLB , II LLB , III LLB, DTL , DLL& LW, I LLM , II LLM, MLL 
Roll Number 
Email ID 
Mobile Number 
Exam Fee : Open (Student Can Enter any amount) 

  1. PRN has to be verified with our Student / Admission Database  
  2. After submitting the form it should show a preview of what they filled and then show buttons to go back and “Pay Fee Online” 
  3. Go back will take student to form in edit mode and allow them to edit anything. 
  4. “Pay Fee Online” will allow them to pay via “SBI Online Payment” 
  5. Once payment is done they should see success message on screen with the download receipt option. 
  6. After success two email notification will b generated one to student and other to college with all the entries student filled. 
  7. “Download Receipt” button will allow student to download the PDF form of the receipt. Format is given.
