PhD admission form

Please check the admission form filed mention in attachment. There is another attachment which will show the details (Edited by client)

This form we need like we have rest of the admissions, means will provide some form id to take to verify the details and then show the form, rest admission flow remains same, just the form details and receipt details will be updated. 

Not urgent once all the ILS tasks completed then take this one.

    1. Form fields are done as given attachment are remaining part is same as like other form.
    2. Need to add merchant code when we start admissions.
    3. PDF format is changed as they mention check attachment for PDF format.
    4. any other changes in this form do mention in flow.

  20. yes on demo pdf code is having error so i uplod only form on production which admissions are closed.
