Teachers Feedback year wise

I have attached the feedback form that need to develop year wise.
The Year will be static below /after the title. The form data will store and will show the feedback report as like other.

The layout is explained in the form.

    1. The form field number and the form field sequencing is not matching in feedback report and survey report. The form field should be similar in all the places

    Correct the spelling of the Neutral where we used in form, pdf, feedback and in survey report. Now it is Netural

    On demo server we done with this sequencing .
    we did this by hard coding the options only for teacher feedback survey report.
